According to Article 24.1 of the Personal Data Protection Act of 29 August 1997 (Journal of Laws of 2002, No. 101, item 926, as amended) we would like to inform you that the Controller of your personal details is HIFU Clinic Centrum Leczenia Raka Prostaty, Praktyka Lekarska Dr n. med. Data Safety Controller
At the HIFU Clinic Prostate Cancer Treatment Centre, a Data Safety Controller appointed with a decree of the Personal Details Controller is responsible for supervising correct processing of Customer details.
