What is the purpose of the post-procedure consultation in prostate cancer treatment?

The aim of this consultation is to monitor Patient's health after procedures used for treatment of prostate cancer.

Regular follow-up consultations are necessary for correct prostate cancer treatment.

What does the post-procedure consultation involve?

After a selected treatment method is applied in a prostate cancer Patient, his health should be monitored for many years, using:

  • sensitive biochemical parameters;
  • diagnostic imaging;
  • verifying procedures.

A regular monitoring of PSA levels is necessary, but increasingly often, also other modern molecular tests are performed, at frequencies and with threshold values specific for a given treatment method. The tests should be performed at various time intervals depending on disease parameters, age, initial and measured PSA levels and their dynamics.

When during monitoring of PSA levels or other parameter its increase is noted, this may indicate a cancer recurrence. In such cases, our Centre and laboratories cooperating with us use advanced imaging methods where individual techniques vary in terms of their advantages and sensitivity:

  • multiparameter magnetic resonance imaging;
  • 68Ga-PSMA-PET/MRI;
  • choline-based PSMA-PET/CT;
  • whole-body magnetic resonance imaging
  • bone scintigraphy.

Frequently, the local condition post treatment requires histopathological verification of material collected during a MRI-TRUS fusion biopsy. This test is already available at our centre.

Who should attend a post-procedure consultation in prostate cancer treatment?

We recommend the post-procedure consultation to all Patients who underwent a therapy for prostate cancer.

Additional information

A change or auxiliary therapy may be required after the first-line therapy. An increase in PSA may indicate a biochemical recurrence, significantly preceding a clinical recurrence and appearance of symptoms. Procedures that may be used after the surgical treatment (radical prostatectomy) include salvage radiotherapy, targeted HIFU for recurrent cancer, or hormonal treatment and chemotherapy.

Suspected recurrence found, for example, in lymph nodes, using diagnostic imaging after a radical therapy, in some Patients can also be treated by selective lymphadenectomy using 111In-PSMA radioguided surgery. This therapy is conducted at our centre in cooperation with a nuclear medicine unit. Also after external beam radiation first-line therapy, a salvage surgical treatment, targeted HIFU for recurrent cancer, and brachytherapy are possible.

In the event of disease progress during the hormonal therapy, the treatment can be modify, for example, by initiating an anti-androgen (a medicine blocking receptors specific to androgenic hormones in prostate cells) therapy or starting chemotherapy. We provide further-line hormonal therapy with medicines normally not available in other medical centre, although many of them are subject to refund restrictions, consultations with specialists, and summary information sheets concerning previous treatment provided by the Patient, medical interview and possible additional tests that may be recommended after the consultation.
